Sorry, but that's the most likely outcome of any of the routes. Lilly and Hisao starts their own life together as teachers, maybe get a kid or two (she's the motherly type, after all) and Lilly finally gets that family she's always longed for, but then Hisao suffers a certain onomatopoeia at the age of 28 and Lilly becomes an alcoholic widow.
You're skipping the final act, where a globetrotting Hanako comes home to find her friend a shell of her former self, and resolves to help her recover from her grief the way she once was helped. She succeeds, and they live out their lives as heterosexual life-mates raising Lilly and Hisao's kids.
Alternative end, just for you: Hisao's condition is cured by the tears of a beautiful unicorn, and they all live happily ever after and have many adorable children. Hanako and Akira are godmothers.
But they used a condom! I mean, I know condoms don't prevent pregnancy 100% of the time all of the time but what's the point of contraception if it isn't going to actually prevent conception?
